Navigate to Platform > Integrations > Slack (admin only) and click Add destination. Give it a label and paste in an incoming webhook URL from Slack - each destination posts to whichever channel that webhook was created for. The webhook URL is masked once saved; leave it blank when editing to keep the existing one.
Each destination has its own list of events to send: new lead created, new ticket created, deal won, org health declined, invitation accepted, and custom form submission. Tick only the events you want in that channel. A destination with nothing ticked stays quiet, and you can mark a destination inactive without deleting it.
Custom form submission is a two-gate event: the Slack destination must have it ticked, and the individual form must also have "Include submissions in Slack notifications" turned on in its own settings (see Building Custom Forms). Both need to be on before a submission posts, so turning on the destination event alone won't flood a channel with every form you own.
Click Send test next to a destination to post a real test message to its channel immediately, so you can confirm the webhook still works before you rely on it.
